Have you and your partner been trying to conceive for some time, or have encountered fertility challenges? Consult with our Doctors to navigate your concerns about your fertility by helping you organise fertility testing via your local pathology service.

Pathology Request Includes: FSH, LH, FSH:LH Ratio, Oestrogen, (Day 21) Progesterone, Testosterone, SHBG and Anti-Mullerian Hormone* - Optional add-on test

How long have you and your partner been actively trying to conceive for?

Do you have a family history of infertility/difficulty conceiving?

Do you have irregular periods?

Do you suffer from very painful periods?

Have you been pregnant before?

Have you had two or more pregnancy losses/miscarriages before?

Have you ever been diagnosed with a Sexually Transmitted Infection (STI) before? E.g. Chlamydia, Gonorrhoea

Would you like to include AMH (Anti-Mullerian Hormone) in your fertility testing? This is an optional test as it is NOT covered by Medicare and costs $90.

After reviewing your order, you will receive a link within 24 hours to schedule a Telehealth appointment with one of our doctors, who will then provide you with your Pathology Request Form.
